family names, phone numbers (home and cell), mailing address, email, birthday
and anniversary information.
Please
check your information on the sample pages in the parish hall, if it is okay –
please indicate that you’ve checked with a check-mark or initials. If changes
need to be made, just cross out and correct the information. Thanks
